Self Pay Rates

Initial Session (60 minutes)- $220
Following Sessions- $200

 

Insurance

Accepting most major insurance plans, including:

​​

  • Aetna

  • Blue Cross Blue Shield

  • Cigna

  • United Healthcare/ Optum

  • Lyra EAP

  • Loveland Vouchers

  • Out of Network

​

If using Out of Network benefits, I will provide you with the appropriate paperwork that can be submitted to your insurance carrier.  Some insurance plans may reimburse as much as 60%- 80%.  However, please call your insurance company and ask about your current benefits and coverage.

Cancellation Policy

Your session time has been reserved for you, and I require at least 24 hour advance notice if you are unable to make it.  If notice is not provided at least 24 hours in advance, you will be charged the full amount for the missed session.

*Payment and Co-pays are due at time of service.
